HOW TO APPLY ONLINE FOR YOUR HOISTING EXAM
Go to the following website: https://www.mass.gov/hoisting
Click on "Apply for a Hoisting Engineer License"
Then click "Apply Online"
Create an account
Once your account is created and you login, you will be able to apply for a hoisting license b click on the
"Apply For A New License / Exam" option
Things you will need to upload in JPG or PDF format
- Driver's license, Driver's license learner's permit, or a valid Massachusetts state ID
- DOT Medical Certificate, Intrastate Medical Waiver from the Massachusetts Registry of Motor Vehicles, or ANSI/ASME B30.5 Medical Qualifications Form
- 2" x 2" photo (Massachusetts residents may elect to authorize OPSI to access their RMV photo)

- What license are you applying for? Drop down and choose "Hoisting Engineer"
- Are you looking to add a restriction to, upgrade an existing license? Licenses must have a status of "Active" to add a restriction. Drop down and choose "No" if this is your first hoisting license. "Yes" if you already have an active license
- Are you looking to renew your license? Drop down and choose "No"

Step 8 - Requirements - will ask for a photo or scanned image of your Driver's License or MASS ID as well as your Medical DOT information and scanned image
Step 9 - Hoisting Engineer License Photo - If you are a Massachusetts resident click Yes and authorize your RMV photo otherwise you will be required to upload a professional 2" x 2" passport size photo
Step 10 - Accommodation
Step 11 - Signature - BE SURE ALL OF YOUR INFORMATION IS CORRECT - Check I Accept, Click Finish.
