HOW TO APPLY ONLINE FOR YOUR HOISTING EXAM
Go to the following website: https://www.mass.gov/hoisting
Click on "Apply for a Hoisting Engineer License"
Then click "Apply Online"
Create an account
Once your account is created and you login, you will be able to apply for a hoisting license by clicking on the
"Apply For A New License / Exam" option
Things you will need to upload in PDF format
- Driver's license, Driver's license learner's permit, or a valid Massachusetts state ID
- DOT Medical Certificate, Intrastate Medical Waiver from the Massachusetts Registry of Motor Vehicles, or ANSI/ASME B30.5 Medical Qualifications Form
- 2" x 2" photo (Massachusetts residents may elect to authorize OPSI to access their RMV photo)
Opening Questions
- What license are you applying for? Drop down and choose "Hoisting Engineer"
- Are you looking to add a restriction to, upgrade an existing license? Licenses must have a status of "Active" to add a restriction. Drop down and choose "No" if this is your first hoisting license. "Yes" if you already have an active license
- Are you looking to renew your license? Drop down and choose "No"
HINTS To Help You Deal With The State
- You are becoming a Hoisting Engineer.
- Restrictions are the Licenses your are applying for - examples - 2A, 1C, 1B.
- You must answer all questions and apply perfectly in one setting. YOU CAN'T START OVER. If you don't finish your application in one session, you will have to call the state and have them mail you a paper application which will take more time.
- Have all the required documents saved and named prior to applying. Save your receipt after applying.
- Use a PC or Laptop when applying - Do NOT use a Mac, iPad, or phone.
- You can NOT pick your test date, time, or location - the state will provide that.
- Do NOT lose the letter from the State. You will need to present that on test day.
- Do NOT bring your phone or talk to other testers in the test location.
- Keep the letter they give you on test day. (I GOT 100%!)
- State fees are $75.00 per test. The 2A/1C is two tests and will cost $75 per test.
- Before applying, be sure your DOT card has at least four (4) months left before its expiration date.

Step 9 - Hoisting Engineer License Photo - If you are a Massachusetts resident click Yes and authorize your RMV photo otherwise you will be required to upload a professional 2" x 2" passport size photo
Step 10 - Accommodation
Step 11 - Signature - BE SURE ALL OF YOUR INFORMATION IS CORRECT - Check I Accept, Click Finish.
